After tightening, have you checked to see if baseplate is/ seems secure? Is there a lock washer installed? Is the wing nut loose when you finish, or did it just slip? Would a star washer help? Would a star washer on the (other) end of the bolt also help? (If a washer is missing, it might effectively make the bolt too long?) When tight, is the nut bottoming out (running out of thread) or is thread remaining?
