When we did this at my parents home, we used 3 2 x 4's caulked and screwed together. The 4 1/2 inches between them seem to be about the closest we could get them with out the nailing flanges over lapping.
We did not need to worry about the header. as this was an original opening from when the home was built. We took out three windows, and put two wider ones in their place.
put caulk between the 2 x 4's because there was no way to insulate, and the caulk will help eliminate air flow between the boards.
