The tile wall is not painted.  I'm looking to paint it.

There are special primers and paints formulated for ceramic tile.

The grout isn't for cracks or holes but for the spaces between the tiles.  The idea being to paint over the tile and have it re-grouted to maintain the tile "look".

There are marker style grout line painters but I have a feeling that would be very tedious and not very appealing in terms of uniformity... being done by hand.

dont have to use grout to fill cracks and holes since you are going to be painting over the
patched areas again.  IE. find a good putty that will stick to the material well.

The big worry is what kind of paint is on the tile and whether it will serve
as a good base for your nest top coat.  If not then you will have to find a primer that will work 
with the paint on the tile first and then apply the top coat.   People use tile because it is
a thousand times tougher than paint and it has to be totally beyond repair before anyone
would consider painting tile as painting is cheaper and faster than ripping out all the old
tile and replacing with new.   If the tile is in good shape.... ( I dont care what the color )
anyone that suggests that you paint over them for purposes of fashion.... 
Shoot them between the eyes immediately.

Same as anyone who ever suggests painting over exterior brick.   In both cases you
are taking a surface that will last a hundred years for no cost and replacing with a surface
good for a few years at best and requires costly touch-up ongoing forever.
