Great detective work, and that looks very similar to the one I recall seeing, but when and where I have no idea. I fired off an e-mail to Lee Valley tools to see what they might come up with, and have another catalog on my reading stand from another company that also carries a superb line of hand tools that I plan to query as well. I grabbed that photo you sent and will send it along as well. That must have come up on E-Bay since the last time I looked, but I doubt I'd have spent quite that much for it. Wonder who the original manufacturer was, it either had not seen much use or was made of some good steel,

I'm sure I've seen one of these or something very similar some place but can not find one to save me. It is a hardened steel plate that is mounted to a bench or table and used to score dowels for gluing. You just drive the dowel through its corresponding hole with a mallet to score dowels of any length. The plate I have drawn to show here is for 1/4, 3/8 and 1/2 inch dowels but the ones I think I have seen had more holes covering a variety of sizes.
