You need a sediment filter before everything else (softener and iron filter). You need it in the main water supply line as it enters the house. A carbon filter you mentioned is for taste filtration, not anything else. It goes to your drinking water (kitchen cold water faucet or ice maker). Not filtering (sediment filter) the water into the water heater and softener allows sediment build-up and clogging.

Cold water lines are not a problem. This problem may have started when I installed a new water heater but I am not sure. My water source is a well with very high iron content. I have a good iron filter and water softener. I installed an inline carbon block filter on my shower head but it plugged up in short order.
