[DIY] Floor tiles

 

A year ago, I put in ceramic floor tiles in my kitchen and bathroom. In 2 spots, the tiles sound like they grind and the grout keeps coming out. I put new tiles and grout in twice and it keeps happening. Is there a product that will not do this? The floor is level in both places also.
How can I fix this so it will not happen?
